Future appointments can be scheduled by calling 214-826-2151 or by printing the Appointment Request Form, filling it out and faxing it to 214-826-2196. Confirmation of your request will be received within one business day.
In order to expedite your check-in, NEW PATIENTS should submit the New Patient Registration and Patient History Form with the Appointment Request Form.
