Physicians refer their patients for Pre-Surgical Clearances/Screenings by faxing the referral to (214) 420-0397. The referral should include a list of desired screenings, patient information and the surgical procedure to be performed. Our appointment coordinator contacts the patient with an appointment. Workers’ Compensation and most private insurances accepted. |
